Scholarships in the amount of $500.00 will be awarded to selected deserving students attending an accredited vocational/technical school, community college or four-year institution. The scholarship is only awarded once.

In order to complete this application, the student will need:
- Personal Information
- High School Information
- College/University Information
- Two Personal References
- Community Involvement / Volunteer Service Hours
- Honors, Special Achievements, Awards, & Extra-Curricular Activities
- Recent Photo (Headshot)
- Electronic Transcript(s)
- Proof of Citizenship (Certified copy of birth certificate, US Passport, or Naturalization papers)
- Letter of Recommendation
